Hospital in the Home (HITH) specialises in providing acute, admitted clinical care within a bed substitution and emergency department diversion model. Our dynamic and motivated staff thrive in a fast-paced environment where they are key members of a multi-disciplinary team who are experts in the @home environment.

The physiotherapist scope of practice includes (but is not limited to) cardiorespiratory, orthopaedic surgery, functional decline, complex comorbidities, and age-related needs. Being an independent practitioner with self-direction and an individually-managed caseload is expected, along with the provision of clinical advice to the interdisciplinary HITH team.  The role may include direct supervision of any physiotherapy students working within the HITH

Duties include:

  • Provide optimal physiotherapy management to patients in a variety of settings such as home, ward or clinic through effective assessment, treatment, evaluation, liaison, and discharge planning.

  • Provide high quality clinically appropriate, comprehensive allied health therapy to patients and carers in their own homes, aligning with the patient’s expressed goals of care and healthcare needs, and considering the effects of the patient’s health issues on their life and wellbeing.

  • Demonstrate sound clinical knowledge of evidence-based practice, research and new developments, including innovative technologies.

Classifications for this position will range from VB1 – VB4 ($88,467 - $101,691 per annum, pro rata for part time).

Your Contribution

  • You will have broad and relevant clinical experience ideally including community based programs

  • You have demonstrated ability to work effectively within a multidisciplinary team

  • You have effective interpersonal skills including problem solving, negotiation and teaching

  • You are experienced in working with patients with multiple comorbidities that impact safety, function and independence

  • You are passionate about continually improving and have solutions focused approach to improve service delivery

  • Current National criminal history check, or willing to obtain

Hospital in the Home

HITH is a bed substitution model that provides the equivalent of hospital-based services within a patient’s home. HITH also provides care to patients admitted to the Cottage, a five-bed short stay facility providing HITH care, to people experiencing, or at risk of homelessness. The team of medical, nursing and allied health staff enable earlier supported discharge from hospital and diversion from hospital where possible.

The position may be based at any of the St Vincent’s Hospital Melbourne sites, and requires staff to undertake home visiting duties.
