About the Role

The primary purpose of the role is to provide best practice clinical care to HITH patients with indications for physiotherapy assessment and treatment. The scope of practice includes cardiorespiratory, orthopaedic surgery, functional decline, complex comorbidities and age-related needs. Prioritisation and management of individual caseload is expected, along with the provision of clinical advice to the interdisciplinary HITH team.  The role may include direct supervision of any physiotherapy students working within the HITH team. 


Classifications for this position will range from VB1 – VB4 ($88,467 - $101,691 per annum, pro -rata).

Hospital in the Home (HITH) is a progressive medically-led, multi-disciplinary team delivering acute, out-of-hospital care via a bed-substitution and Emergency Department-diversion model.

HITH specialises in providing comprehensive and personalised care to patients with acute clinical needs.  

This may include care of: 

  • patients with drain tubes, who require pre- and post-operative anticoagulation and post-anaesthetic recovery 

  • patients requiring intravenous therapies such as electrolyte replacement, iron, antimicrobials, steroids, fluids and total parenteral nutrition  

  • patients with complex wound care, including negative pressure wound therapy 

HITH spans a geographically diverse map and within a variety of settings and accommodation: 

  • The Cottage, a five-bed short stay facility for people experiencing homelessness and social vulnerability (located at main campus – Fitzroy) 

  • HITH Clinic 

  • Private homes

The position may be based at any St Vincent’s Hospital Melbourne sites and requires staff to undertake home visiting duties for HITH clients.

3. Position Duties

The delivery of the highest standard of physiotherapy services to the HITH team as directed incorporating the mission and values of St Vincent’s Health Australia.

  • Provide optimal Physiotherapy clinical management of HITH patients including effective assessment, goal setting, intervention, liaison and discharge planning

  • Provide guidance to the HITH team regarding indications for physiotherapy intervention

  • Communicate assessment and treatment outcomes to nursing, medical and other allied health staff via documentation and relevant meetings.

  • Communicate effectively with inpatient multidisciplinary team to facilitate efficient continuity of care

  • Work collaboratively with HITH team members to ensure patients receive best practice care to meet their needs. Actively engage in building relationships and communication with other community-based providers who may refer to HITH, or receive discharge planning referrals from HITH

  • Participate in relevant HITH meetings - daily clinical ward round, team meetings, quality huddles

  • Model and demonstrate effective interdisciplinary practice

  • Clinical supervision and teaching of physiotherapy students as required

4. Incumbent Obligations
 

General

  • Perform duties of the position to best of their ability and to a standard acceptable to SVHM.

  • Take all necessary care and precautions when undertaking clinical procedures.

  • Complete annual clinical competencies.

  • Maintain skills and knowledge necessary to safely and skilfully undertake clinical work.

  • Consult with peers and other experts and refer to other healthcare workers when appropriate and in a timely manner.

  • Collaborate and clearly communicate with patients/clients and the healthcare team.

  • Participate in clinical risk management and continuous quality improvement activities as part of day-to-day work.

  • Ensure consumers receive information in an appropriate and accessible format.

  • Actively support consumers to make informed decisions about their treatment and ongoing care.

  • Ensure consumers are aware of their rights responsibilities and how to provide feedback.

Health and Safety

  • Protect the health and safety of self and others, complying with all health and safety related policies, procedures and directions

  • Complete required Fire and Emergency Training annually

  • Complete required Workplace Culture and Equity Training annually

  • Attend general hospital orientation within 3 months of commencement

  • As required, comply with fit-testing and PPE requirements

  • Participate in reporting and analysis of safety and quality data including risks or hazards,

  • Report any hazards, near misses and incidents (regardless of whether an injury occurred or not) into Riskman

  • Identify and report any variance to expected standard and minimising the risk of adverse outcomes

5. Selection Criteria

5.1 Essential Registration, License or Qualification Requirements

  • Degree in Physiotherapy

  • AHPRA Physiotherapy Registration

  • A current Victorian driver’s license

5.2 Other Essential Requirements

  • Broad and relevant clinical experience ideally including, bed-based substitution models of care and/or community based services

  • Demonstrated ability to work effectively within a multidisciplinary team.

  • Demonstrated highly effective interpersonal skills including problem solving, conflict resolution, negotiation and teaching.

  • Experience working with clients with multiple comorbidities and complex care needs that impact safety, function, and independence.

  • Demonstrated committeeman to continuous improvement and ability to contribute to, and implement projects to improve delivery of service

  • Demonstrated commitment to ongoing learning and professional development of self and others

5.3 Other Non-Essential requirements

  • Short postgraduate courses in relevant areas of physiotherapy

  • Postgraduate qualification/s in relevant area/s of physiotherapy
