About the Role
An opportunity currently exists for a Grade 2 physiotherapist to join the Virtual and at Home Department at St Vincent’s Hospital Melbourne, within the Transition Care Program.
The successful applicant will be responsible for providing high quality, overall physiotherapy management to patients admitted within units serviced by the Transition Care Program based at St Georges Hospital Kew.
You will work within a multidisciplinary team to deliver outcomes that are determined by client care plans and goals. The role is also responsible for supervision of students, Grade 1 staff, and Allied Health Assistants and taking a leadership role within the department and other duties as directed/delegated.
Applications from current grade two physiotherapists or suitably experienced grade one physiotherapists will be considered. Relevant postgraduate qualifications would also be highly regarded.

About Transition Care Program
The Transition Care Program is jointly funded by the Commonwealth and State and Territories. St Vincent’s Transition Care Program is resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care.You will work as part of a multidisciplinary team to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.
Classifications for this position will range from classification range VB1 - VB4 ($90,235.60 - $103,724.40 per annum).
Your Contribution
Tertiary qualifications in Physiotherapy
Minimum of 5 years’ experience in a health care setting.
Demonstrated ability to work autonomously and as part of an interdisciplinary team.
Knowledge of and commitment to continuous quality improvement.
Experience with student supervision.
Superior written and verbal communication skills.
Current national criminal history check, or willing to obtain.

1.LOCAL WORK ENVIRONMENT
St Vincent’s Virtual & Home have admitted services across Acute and Subacute streamed Units including Hospital in the Home, Rehab@home, GEM@home, Transition Care Bed Based. Non admitted services include District Nursing, Residential in Reach, Palliative Care@home, Transition Care Home-based and Home Based Allied Health.
This position is part of the Transition Care Program which is jointly funded by the Commonwealth and State and Territories. St Vincent’s Transition Care Program is resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care.
The intent of the Transition Care Program is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.
The target group for Transition Care services at St Vincent’s are:
- Older people exiting from hospital who, via participation in the program, achieve further improvements to their physical, cognitive and psychosocial functioning and an improved capacity for living independently; and/or
- Older people exiting from hospital for whom the focus will be on actively maintaining their functioning while assisting them and their family/carers make appropriate long term care arrangements.
The Transition Care Program is located at the St Georges Health Service in Kew however you may be required to undertake home visiting duties or work from other St Vincent’s campuses.
2.POSITION PURPOSE
An opportunity currently exists for a Grade 2 physiotherapist to join the Transition Care Program. The successful applicant will be responsible for providing high quality, overall physiotherapy management to patients admitted within the Transition Care Program at the St Georges Campus in Kew of St Vincent’s Hospital Melbourne.
You will work within a multidisciplinary team to deliver outcomes that are determined by client care plans and goals, either in a bed based and/or home based setting. The role is also responsible for supervision of students, Grade 1 staff, and Allied Health Assistants and taking a leadership role within the department and other duties as directed/delegated.
The incumbent will be accountable professionally to the Chief Physiothearpist and operationally to the manager of the Transition Care Program. This position may be required to work at any campus as directed by their manager.
3.POSITION DUTIES
- Provide high quality and evidence-based patient centred care.
- Demonstrate advanced clinical knowledge in the area of aged care and care of the older person
- Provide appropriate, timely and effective clinical management to all patients through assessment, treatment, evaluation, team liaison and discharge planning.
- Complete client goal setting in consultation with patient/family/carers as appropriate. Goals are to be functional and measurable.

- Demonstrate high level decision making skills to assess and reduce risks to complex clients by working autonomously and with the support of supervisors to address specific issues

- Assess for and arrange for supply of equipment to clients to improve safety, maximise functional ability, mobility and independence
- Active participation in therapeutic group work as required.
- Ensure all documentation, client records, statistics and activity recording are completed and managed in a timely manner consistent with SVHM, SVVH and TCP policies and procedures, legislative and regulatory requirements.
- Manage workload with a high level of autonomy and responsiveness to program and organisational drivers.
- Actively participate in supervision, professional development and performance review
- Contribute to the skill development of the Allied Health Assistants.
- Participate in quality and continuous improvement initiatives, team processes and service development activities
- Complete annual clinical competencies, required learning and accreditation inccordance with professional and legislative requirements.

6.SELECTION CRITERIA
6.1ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS
- Tertiary Physiotherapy degree.
- Minimum of 5 years’ experience in a health care setting
- Current AHPRA Registration
- Eligibility for APA Membership
- Pre-employment Victoria Police Safety Check
- Victorian Drivers License
6.2 OTHER ESSENTIAL REQUIREMENTS
- Recent experience in and knowledge of physiotherapy management in an acute or subacute health care setting;
